a atr 72 is approaching phakuru airport in nepal for yeti airlines in the himilaya mountains with a passenger filiming the flight as they decend through 12500 feet for the airport with an experienced crew in clear conditions and they expect a visual landing the captain is the widow of another pilot but the captain a woman isn’t expereinced in the aircraft. the crew are told to expect runway 12 for landing and are cleared for the approach. the passenger who is live streaming is indian and this is highly unusal. they have to avoid mountains in the area but they are visiual and are in sight. less than two minutes from the airport and configure the landing gear and disconnect auto pillot and start a cruical turn and set the flaps and only a minute form touchdown go through the checklist. just as they are about to land the fo realises the engines have no power all of a sudden and they won’t get any power 400 feet from the ground. the crew realise it is serious even pushing the engines to the limit but there is no torq a key indication the fo takes control as the more experienced pilot and they stall and crash just short of the runway and the crash it self somehow was live streamed but the streamer the pilots and everyone else is dead. only 1 body isn’t identified. it was the deadliest crash of the airline. the wreckage and the black boxes are sent to france and singapore due to nepals limited facitilies. they look at the orientation of the aircraft and find the left wing hit the ground first. the crash itself going viral in a video showing the left wing had already lost lift and was banking at the time of the crash indicating a stall. the design of the approach is also a concern as they were on their last left turn making them worried but it isn’t the crew mishandling the turn. matience checks out and the controller didn’t know anything and he didn’t know anything was wrong till the crash itself and an alarm in the tower the controller even asks the investigator what he thinks happened. a witness who is an enginneer reports the propeller spinning very slow at the time of the crash a key lead and it sounds like engine failure. investigators notice on a propeller hub that it looks feathured which means it isn’t working as they aren’t producing thrust and was stuck there but not all the propellers are which makes them think is an automatic resposnse to a failure. but the auto feather didn’t activate meaning a pilot did. they look at the engine controls they find the witness marks that the controls were positioned at that position on the left side but not the right. the fdr is required to settle the debate and the results are shocking with the engines at idol but key indications dropped a minue before the crash. they need to figure out how they made the mistake. they look at the cvr and find they were feathured when the auto pilot was disconnected and were on decent but figure out the pilot did it when he was supposed to change the flaps. they look into the pilots and find they weren’t drunk at the time of the crash. they look into fatigue and find they had adequette rest and no medical issues and find the controls are confusing. think they could have messed up which is which. the last minute runway change which at a new airport is supicious as the fo was instructing the captain about the new airport and also he made the error. even though they could have recovered the crew didn’t follow the checklist. the crew knew the engines had problems but didn’t figure out the real problem.
