air blue flight 202 is on its way to islambad and is on decent to the airport with 146 passengers and 6 crew on the a321 and are expected to do a circling approach to runway 12 with the experienced captain named chowdrey who is near the end of his carrer. the fo is a former air force leader. the plane is cleared to 3900 feet. islamabad is surrended by the himalyas and they must approach from the south due to the winds they must do a complicated visual approach. thick clouds are causing problems and diversions. they are less than 10 minutes from landing and are on decent lowering the gear and are at the minimums heading 352. they launch their first turn and need to keep visual. the controller is expecting to see the lights when he gets a call but they are told their approaching the no fly zone and are told they are north and turn left. and are told to do a missed approach but they don’t respond for a while and the controller is nervous though but they aren’t turning and are headnig for the mountains and are struggling to turn left. the gpws sounds saying terraibut the plane can’t turn and are out of control even the passengers are aware heading towards magala hills and they are decending at the wrong moment crashing killing all on board 7 miles from the airport at an alittude of nearly 3000 feet. rescuers find complete devistation, the engines were fully functional at the time of impact. and the tabilizer and gear was normal and were climbing at full power. the weather is a suspect but wasn’t dangerous but challenging but they should have known the hills and should not go too far north of the runway that far. the controller says the pilots understood the approach which requires visiual the whole time. the controller waa worried about the plane not being spooted flying and asks if they had visual with the ground they said yes but kept flying away and he had warned many time sbut the pilots ignored it or didn’t react. they look inton the pilots and the captain had over 40 years flying but not much airbus experience the fo being an ex fighter pilot and had less experience on the airbus. they look at the approach and after the first turn they flew the wrong way into the mountains. the gpws should have warned them a minute before the crash and it did but they misreacted to the 21 warnings. the investigation is supprised that the captain was putting way points into the system to do the approach and are told about the pia 737 landing safely and the china southern diversion. but they start the turn late. the captain switches to nav mode which is not what they are supposed to and is violation of procedures the fo is concerned about this thinking the captain has a visual and they don’t and he ignores the fos warnings and the captain falls to turn away which concerns the controller and the fo even asks the captain what to tell them and the captain says they are visual with the ground which is not helpful and the captain ignores the fos warnings and doesn’t react properally. the fo keeps warning him about going down and the terain but he ignores it. the captain had put an alittude in that was illegal in these conditions without the runway in sight which is not legal. the captains decision to switch modes on autopilot was a big error which was switching from heading to nav mode which put them dangerously close to the mountains into a no fly zone. the captains directions werne’t right due to the autopilot because of the mode and didn’t switch it and when he corrects it the plane turns right not left due to his last imput which he should have done manually leading to questions on the fos actions and look at the start of the flight. the captain asks a question that confuses the fo with a question and critiercises him from less experience with berating which is not crew resource managemnt it is to the point of bullying which discoraged the fo to the point of torture with the questions. they also find the captain had diabettes and hyper tension as well as difficult but not at this level. they consider the fos background and a hidden rivaliry. the berating meant the fo couldn’t do anything without being risked of being berated.
