an aeroperu 757 prepares to depart lima on october 2 1996 for santiago chillie unde the command of captain eric schriber and fo david fernandez. the fo is doing the radio communcations they take off from runway 15 with 61 passengers and a crew of 9 leaving just after midnight. in the cockpit the fo spots a problem with the altimiters being stuck as well as a windshear warniing confusing the crew which is false then they lose the speed indication. the crew tells the controller that they have an emergency and are told to change frequencies. the captain takes over the controls and request ils approach back at lima and a turn of heading 330 and are heading north and the controller is confused about the altitude and haven’t turned towards the airport as they are staying away from the coast and holding what they think is 10000 feet this is to avoid mountains. no one in the cabin knows whats happening. they start turning north and decide to decend with lower engine power which in this situatiion won’t help as they don’t know their alititude the speed indication is though going up. the speed is confusing and the crew activate the speed breaks which will make things worse and they get an false overspeed warning and than a stall warning which is confusing but maybe right. they request help from another plane and they get info that a 707 will come and try and save them they want to intercept the ils. and they think htey have speed under control and at a safe altitude the later is wrong. the controller says 9700 feet as they get a terrain warning as they hit the water dooming the plane as they turn over into the water killing all 70 people instantly 48 miles from the airport. most fo the bodies are in the bottom of the ocean. the radar track confuses the investigators and its confsuing the altitude. the atc transcript reveals to investigators that the speed and altitude problems were reported. they know of a bergan air crash 8 mounths earlier which makes them think its similar. the wreckage is located at 680 feet in the water the blackboxes are found and the ntsb will look at them and they find they are blocked with silver tape the same color as the plane which would make it difficult to differentiate. miatnece records reveal they had a bird strike on the previous flight which means it had to polished which would lead to problems if not removed. investigators interview the line machnic was sick so a lower worker did it which is easy to miss with the type of tape both he and the captain missed the tape. it was only 200 feet whent he problems started the captain relying on the falty data they stop using the instruments and the pitch and power procedure. they are confused about auto throttle and focus on the failure warnings which were mostly false and meant the autopilot was useless. they look at how they tried to return to lima and make the mistake of slowing down when they think its going faster. the captains confused and the overspeed makes things worse they activate the speed breaks and power cut means they were stalling which leads to the loss of altitude and the ikely crash but they also head further out to sea which is not going to help and the terain alarm is why and was the only correct alarm. thinking they are safe over water which is stupid and not really true. and for a minute they get to an ok altitude but the decision to turn back to lima is right but not to decend yet. the controller reports what he saw on the radar screen which only would lead to more confusion which gave the crew false hope. they look at if they could have saved the plane if they checked the radio altimiter and once they hit the water banking they are doomed but the damage from the inital hitting water was too much. they look into the qhr and aeroperu training and find nothing that could help them which lead to the training working against them.
